Here are the next steps…
Step 1) Patient Intake
To become a patient at EVOLVE and have the initial testing covered by OHIP, you will need a valid OHIP card. There is only one test, which is not covered by OHIP (AMH is a $125 fee).
Once you are ready to move forward, simply contact our Patient Care Coordinator, Halle, at info@evolveeggfreeze.com, and she will get you set up with a patient file and a phone intake appointment with our nurse practitioner.
Step 2) Fertility Testing
Once you receive your fertility testing instructions, you will wait until the first day of your next period to set up testing. If you have an irregular or absent cycle, you will contact our day 1 team to schedule your testing on a random day. This testing will include blood tests and an ultrasound, and will give us a picture of your current fertility health.
Step 3) Pre-Freeze Clarity Call
While you wait, we invite you to book a complimentary 15-minute Pre-Freeze Clarity Call with one of our egg freezing–focused NDs. This appointment walks you through what’s happening in the pre-freeze window, and how small shifts in things like nutrition, lifestyle, and stress can help support egg quality in the time leading up to your egg retrieval. Step 4) Follow Up with EVOLVE Physician
After your pre-treatment tests are complete, you and your EVOLVE physician will meet to discuss your results.
This will involve predicting how many eggs you’re likely to get, the ideal number you want overall, and planning for your unique stimulation (treatment) cycle.
With this plan in place, you are ready to start! At this step, you are really in the driver’s seat. Some patients start right away, while others schedule it within 6 months or so. Note that if you postpone it later than 6 months, you may need to repeat your bloodwork and ultrasound. Alternatively, we have patients who still have more questions, want to meet with a therapist and discuss their emotional needs, or would like to consult with someone on the nutritional side.

Fees for Your Egg Freezing Cycle
Pre-treatment blood and ultrasound testing is covered by OHIP.*(excluding AMH testing)
- AMH test is $125 (not covered by OHIP)
- First egg freezing cycle is $8,800*
- Subsequent egg freezing cycle is $7,800
- Medication ranges from approximately $5,000 to $10,000, depending on your unique needs.
Annual Storage Fees (Select ONE):
- $2000 for 5 years, payable at the time of freezing (cycle fee $8600 + $2000) or
- $780 per year starting 1 year after freezing
*This includes blood tests, ultrasounds, egg retrieval, cryopreservation (freezing) of the eggs, as well as the first year of storage (if opting for the yearly storage rate). This fee also includes a Bird&Be Starter Pack and a VIOLET report on the viability of each of your eggs.

Financial Options, Extended Plans & Taxes
Extended Health Plan
Most company benefit plans will cover the cost of medication. Additionally, we are working with many large companies to add fertility treatments to their Benefit Plans.
If your company covers fertility treatments, this will cover the cost of the egg freezing cycle. Before treatment begins, you should review your Company’s Benefit Handbook and contact your plan’s provider. If you require any assistance in navigating your insurance coverage, we have a financial coordinator who would be happy to assist you.
Questions to ask your plan provider:
- Are fertility medications covered?
- What percentage are they covered?
- What is my annual or lifetime maximum? (Your EVOLVE nurse can provide you with a detailed list of medications if your insurance provider requires this.)
- Does my company cover fertility treatments?
- Does this include Egg Freezing?
- Can you email me a list of the services covered and my lifetime maximum under my plan?

Medical Expense Tax Credit (METC)
The Canadian and Ontario governments both provide tax write-offs for medical procedures and out-of-pocket drug expenses. The tax credit allows you to deduct the cost of your treatment, drugs, and medical supplies against your income. So, while you are responsible for the full amount of the treatment up-front, you will get a credit on your income taxes.
In general, for those who make $75,000 or more the rebate is $2,000 and upwards. More information can be found in the guide linked above, or on the Government of Canada’s website.
Ontario Fertility Treatment Tax Credit
Starting January 1, 2025, Ontario residents can claim 25% credit on eligible fertility treatment expenses, up to $20,000 per year, resulting in a maximum credit of $5,000 annually.
This includes costs for egg and embryo freezing (including storage), drugs, diagnostic testing, travel, and IVF cycles. This new credit can be claimed on top of the existing federal and provincial medical expense tax credits, providing extra financial relief for those undergoing fertility treatments. More details are available on the Ontario website.
