Do I need a referral?

You don’t need a referral to speak with an EVOLVE nurse. However, if you’re ready to begin the process, you’ll need a referral from your doctor to cover your consultation with an EVOLVE physician. If you don’t have a family physician, simply contact us to arrange the referral.

Is it covered under OHIP? Can I afford it?

Your first educational visit with our nurse specialist is free of charge. From there, with a referral, you can book an appointment with one of our doctors, which is covered by OHIP. Further testing and treatment is not covered by OHIP. However, extended healthcare plans and tax benefits may be available to help with related expenses. Please refer to our “Fees” page for further details on costs, or speak to our Evolve patient coordinator, who can help walk you through the details.

What should I expect at my consultation with an Evolve nurse?

Your first visit, either virtually or in-person, will be with our nurse specialist. Here you can ask questions to educate yourself on the benefits, costs and risks associated with egg freezing. You’re welcome to have a friend with you - we want you to feel at ease and comfortable. This informal session is all about YOU and there are no wrong questions.

What do I bring to my fertility assessment with an Evolve physician?

Your first appointment with an EVOLVE physician will last about an hour. We will take an extensive health history and do some blood work. You will have the opportunity to ask the doctor questions and plan out the next steps in your journey.

If you have a family physician, you can download our referral form and they will send it to us directly. If not, simply contact us and we will arrange the referral.
